To book an appointment

1. Pay the consultation charges ($325USD)

2. Once payment is complete, simply e-mail shilpa@homeoquest.com with your details and propose two possible dates and times. (See Timings for available times).

3. Fill up the appropriate form below and return via email:

• If the patient is your child please fill in this form
• If you have an acute problem then please fill in this form
• For all other patients with long term chronic issues - please fill in this form
